Patient Information

Our patients are most often referred to us by their vision professionals for highly specialized evaluations, explicit diagnoses and visual prognoses. On occasion, a patient may be self-referred. We will need a copy of all new patients’ most recent medical records for an accurate evaluation. Please bring these with you or have them forwarded to our office before your appointment date.

About Your Appointment

After registering with our patient care coordinator, you will receive a complete ophthalmological examination by a retinal diseases specialist. Upon completion, your examination findings will be reviewed by Dr. Fishman. If any additional testing or study is needed, it will most often be performed at this time, or, infrequently, rescheduled for a future convenient time. Once all tests have been completed, Dr. Fishman will examine your eyes. He will then explain his findings, answer your questions and advise you on his recommendations. New patient appointments may take up to four or more hours and your eyes may be dilated. Please make any necessary arrangements.

What to Bring

Please bring your current eyeglasses, any handheld visual aids, a complete list of current medications and supplements (including over-the-counter medications), your photo ID, any guarantor’s photo ID, applicable medical cards, your insurance authorization determination and your co-payment.


Please check with your insurance carrier regarding coverage for appointments with a medical specialist and/or the requirement for pre-authorization before arrival at our facility. We may be unable to schedule appointments until all required insurance forms have been received. For returning patients, please verify that your insurance coverage is current and has not changed since your last clinic visit.


We appreciate your cooperation in keeping your appointment and arriving on time. If you must cancel your appointment, please provide at least a 48-hour cancellation notification by calling (312) 997-3666.


Parking is available at the UIC Wood Street garage, located at 1100 S. Wood St., immediately north of The Chicago Lighthouse.
