About Your Appointment

You will receive a complete ophthalmological examination by an inherited retinal disease specialist. Additional testing may be needed, and can be performed at this time or rescheduled for a future visit. Expert vision care specialists will review the findings of your examination, answer your questions, and recommend further services, such as low vision rehabilitation, orientation and mobility training, and others.

What to Bring
To ensure your appointment is as comprehensive and smooth as possible, please bring with you:

• Current eye glasses
• Any handheld visual aids
• A complete list of current medications and supplements (including over-the-counter medications)
• Photo ID
• Applicable medical cards and insurance authorization determination
• Your co-payment

Additionally, we will need a copy of recent medical records for an accurate evaluation. Please be prepared to bring these with you or have them forwarded to our office by your referring doctor.

Insurance
Please check with your insurance provider regarding coverage for your appointments with our specialist and/or the requirement for pre-authorization before arrival for your appointment. The UIC Department of Ophthalmology and Visual Sciences accepts most of the same insurance plans as The Chicago Lighthouse. Please contact the Pangere Center Partnership if you need additional assistance or information about insurance coverage.

Cancellations
We appreciate your cooperation in keeping your appointment and arriving on time. If you must cancel your appointment, please provide at least a 48-hour cancellation notification by calling (312) 997-3666.

Parking
Parking is available at the UIC Wood Street garage, located at 1100 S. Wood St. This parking lot is located within easy access to both The Chicago Lighthouse and the UIC Department of Ophthalmology and Visual Sciences.
