Refer a Client for Mental Health Services

We welcome referrals for individuals interested in receiving counseling services through the “Stronger Together” initiative. We offer individual, family and group therapy services through the Bergman Institute for Psychological Support. Our mental health providers have experience working with the visually impaired, blind, disabled, and Veteran communities and are dedicated to providing compassionate support that can meet your client’s unique needs.

To refer a client for services through the “Stronger Together initiative”, please complete and submit our secure, HIPAA-compliant External Referral Form.

Once submitted our Mental Health Care Coordinator, Michelle Lee, will reach out directly to the client for scheduling. We provide a status update to our referring partners both after the client is scheduled for services and when they complete their first appointment.
